Corrections are a big part of our practice...

Most of the time the conversation about correcting inappropriately placed filler or an overfilled area starts during a consultation when a patient walks into our office wanting to improve or enhance a specific area, not understanding that the cause of the problem is filler that was injected previously, sometimes even several years earlier.  Inexperienced injectors often do not completely understand the properties and lifetime of fillers.  They often select inappropriate filler product for the facial area. They the filler inject too thickly, too deeply or shallowly.  This often results in patient looking less than attractive, unnatural, or enhanced.

Botched filler correction treatment is a 2 step process.

First: incorrectly placed filler or an overfilled area is dissolved, then patient comes back for a follow up evaluation.  Possible touch up may be needed to achieve optimum correction.

Second: area is re-injected which is common for lips, or individual plan  


This patient has been getting filler injections from another provider.  Multiple areas of her face were overfilled making her look older and less like herself.  First: filler was dissolved - under eye and cheek bone area, above and below lip area, NLF, jowls. Second: we re-injected some of her features enhancing her beauty.  Notice she looks much younger after correction treatment.

I can give many examples of “botched filler,” the most common being:   

Nasolabial folds (NLF): this area is notorious for being over-injected.  Many patients don’t likedeep folds and shadows in this area and often request that it be treated.    Filler starts to accumulate, making the area look heavy and weighed down.  The patient looks at their face thinking they still don’t like the “lines” and that their face still looks droopy.  The actual problem is that with age, most of us have some narrowing and thinning of the lateral face.  The necessary solution here is injecting an area of the face that has lost volume, which will, in turn, improve the appearance of NLFs. Some dissolving at NLF is necessary to improve facial balance.

Lips: are probably the most frequently botched area.  Lips are so delicate that if filler is placed incorrectly, it can not only distort lip shape, but it will also distort the smile and facial movement.  Botched lip filler can cause asymmetry, incorrect proportion, puffy donut-like padding above the upper lip, visible especially when smiling, a “fish-like” profile, distorted “sausage-like” shape, or caved in corners of the mouth.  

Under eye area: there are two distinct botched categories. 1. The first category has to do with volume or, in other words, overfilling an under eye area.  The patient’s eyes appear small, the lower eyelid does not have a distinct shape and appears as if the cheek connects directly to the lower lash line. It is a very distinct “done” look.  This happens ether due to inappropriate choice of filler for this facial area or overfilling.  2.  The second category is an uneven bumpy appearance or a single bump, even resembling an under eye bag.  This happens when the filler was injected too superficially.  Patient may take notice to the uneven appearance several weeks post treatment or 3+ months post treatment when the body starts to slowly metabolize the filler.

Additional areas that can be “botched” are cheekbones, chin, marionette lines, etc. Any area of the face can start to appear unnatural or throw off the rest of the facial balance.
